Evanston’s Health & Human Services department is conducting a survey to assess the community’s health status. The survey is designed to gain insights on local health trends, and to overall gauge how healthy the community is. Anyone Evanston resident who is above 18 years of age is eligible to take the survey, and all recorded responses are anonymous.
Beyond collecting information from participants, the survey asks respondents what they view as important factors for a healthy community, and how they view the health of Evanston. Questions cover a wide range of topics, including physical health, mental health, household health precautions, and sex.
This survey is part of a larger project called Evanston Project for the Local Assessment of Needs (EPLAN), which aims to improve the health of the Evanston community as a whole through anonymous data.
The survey is available online here and can be completed until August 26, 2015. It will take 10-15 minutes to complete.
